Service Description

  • This service allows the issuance of a medical report on the condition of the patient based on his medical record in the hospital

Eligibility or Criteria

  • The applicant patient must attend in person, if he is alive


  • A first-degree relative of the patient must apply in person, if the patient is alive but cannot attend in person


  • The applicant must refer to the Legal Affairs Department of the Ministry of Health, if the patient concerned is deceased


  • The request must be submitted through the Prison Authorities, if the applicant is imprisoned  


  • The applicant must refer to the Legal Affairs Department of the Ministry of Health, if he has a pending investigation in a legal case


  • The applicant must have a medical record in the hospital

Required Documents

  • Civil ID
  • Patient's medical record

Procedures

Visit the Office of Medical Reports and Commissions in the hospital administration concerned and submit the required documents
Fill the service application form
Receive the report at the specified date in person
