Service Description

  • This service enables users to create a new file to obtain a disability certificate.

Eligibility or Criteria

  • All citizens and residents are entitled to apply to create a medical file

Required Documents

  • A valid civil ID card of the person concerned is required.
  • A recent medical report (in Arabic or English) issued not more than one year ago from a government hospital in the State of Kuwait is required.
  • For non-Kuwaitis with a Kuwaiti mother, a birth certificate for the person concerned, the mother’s civil ID, and a copy of the mother’s nationality certificate are required.
  • If the person concerned is under 21 years old, please attach the guardian’s civil ID and birth certificate

Procedures

The application must be submitted at the headquarters of the Public Authority for Disability Affairs in Hawalli, Block 11, Beirut Street, opposite Al Bayan Bilingual School, or at one of the Authority’s branches during the morning period from 8:00 a.m. until 1:00 p.m.​
It is required to attend the medical committee appointment specified by the Authority. ​

A text message will be sent with the committee's results. 

​If approved, the Authority must be contacted to receive a disability certificate​
