Service Description

  • This service allows the issuance of a permit to accompany a patient in the hospital wards. This service is not considered an absence permission from work except in case of mothers accompanying their children at the hospital

Eligibility or Criteria

  • The patient's medical condition must require the presence of a companion

Required Documents

  • A recommendation from the doctor

Procedures

Obtain the service application form, i.e. Accompanying a Patient Form, from the reception in the hospital concerned
After filling the service application form by the doctor, submit it to the ward supervisor
