Service Description

  • This service allows the business sector (companies and organizations) to add, remove or modify radioactive apparatuses and resources listed in the official license

Eligibility or Criteria

  • The applicant entity must submit an official letter directed to the Radiation Protection Department

Required Documents

  • Official letter from the entity applying for a license to (add - remove - modify) a radioactive resource or an apparatus containing radioactive resources or an ionic radiation apparatus directed to the Radiation Protection Department and stating the required change in the license
  • Technical specification booklet for the radiation apparatus or the radioactive resource or the apparatus containing the radioactive resources and indicating the usage methods and available preventive measures
  • Original architecture plan for the apparatus required installation and operation site and a copy. This plan must define the dimensions and the location of the apparatuses, the type and thickness of the walls, and the occupied areas in the location and their uses
  • Signature authorization of the legal officer
  • Fee payment receipt
  • Copy from the fee payment certificate of the applicant entity to the Public Authority for Social Securities. The government entities and public sector may not submit this document

Procedures

Visit the Radiation Protection Department of the Ministry of Health and submit the required documents
Fill the service application form and have it approved. The radiation apparatus storage and usage locations are inspected
If the request is rejected, an official letter is issued from the Radiation Protection Department stating the rejection reasons

(30) KD Fixed Fees: License of a radiation apparatus in a hospital or a radiation clinic
(15) KD Fixed Fees: License of a radiation apparatus in a hospital or a dental x ray clinic
(15) KD Fixed Fees: License of a radiation apparatus in a specialist clinic
(30) KD Fixed Fees: License of a radiation apparatus in an industrial or other institute
