Service Description

  • This service allows you to issue a replacement for lost/damaged nationality certificate

Eligibility or Criteria

  • Applicants must attend in person
  • Damaged certificate must be submitted to issue a replacement

Required Documents

  • Original civil ID and a copy
  • Copy of the nationality certificate, if available
  • Letter from the applicant
  • Four (4) recent personal photos (4 × 6 cm) with blue background
