Service Description

  • This service enables (GCC) citizens to report and issue a lost / damaged card

Eligibility or Criteria

  • Concerned must attend in person or his / her legal guardian or legal representative    

  • Lost / damaged card form must be signed    

  • New card will be issued after (60 days ) of report    

  • (Certificate good for (60days ) can be issued upon request for (1 KD

Required Documents

  • original passport , and a copy of it
  • Original civil id card for (damaged card) only
  • ( 2recent personal photos according to specifications over ( 5 years old
  • Blood type, If not registered in old card
  • Signed photo form
  • Original power of attorney and original id of the empowered

Visit PACI head office - south surra – 1st floor - citizens and GCC registration office – working hours 8am to 1pm
