Service Description

  • This service allows patients to receive social - financial - psychological services

Eligibility or Criteria

  • Applicant must attend in person


  • Applicant must be transferred by the treating doctor

Required Documents

  • Civil ID
  • Salary report
  • Lease contract
  • Marriage contract
  • Monthly commitments

Procedures

Visit the social services office of the patient's hospital, and submit the required documents
Fill the service application forms. You can request help directly from the social worker during his/her daily check on the wards. You can, also, call the Social Services Office, if you want to request consultation, or ask any of the medical staff members to call for the social worker
