Service Description

  • This service enables users to have a certificate of proof of caregivers

Eligibility or Criteria

  • The person with a disability must be a Kuwaiti citizen or be treated as a Kuwaiti individual pursuant to a decision issued by the Ministry of Interior of the State of Kuwait.

  • This person must have a moderate or severe disability.

  • The caregiver certificate for minor disabilities is issued only to the Credit Bank.

Required Documents

  • Civil ID cards for both the person with a disability and the person requesting to be a caregiver (provided that the addresses on both cards match)
  • Birth certificate of the person with a disability and the person requesting to be a caregiver
  • Disability ID
  • Official notification (social status statement) for the person requesting to be a caregiver and the person with a disability (18 years and older)
  • Certificate issued by a government hospital stating that the person requesting to be a caregiver is medically fit
  • Criminal status certificate showing that the person requesting to be a caregiver has no court rulings for a crime involving dishonesty
  • Certificate stating that there are no final rulings imposing guardianship on a person with a disability from the Ministry of Justice (Judicial Inquiry) for people with mental and psychological disabilities (aged 21 and above)
  • Final custody ruling for a person with a disability in the event of separation between the father and mother
  • Signing the declarations prepared for this purpose for a person of full legal capacity
  • Ruling of legal interdiction or the absence of rulings in the event of applying to be a caregiver to a person with a disability or an incapacity 
  • Submitting a medical report stating the disability of the person who has priority, according to the text of Article 25 (if any)
  • Submitting a death certificate for the person who has priority according to Article 25, if any, along with a list of heirs

Procedures

The application must be submitted at the headquarters of the Public Authority for Disability Affairs in Hawalli, Block 11, Beirut Street, opposite Al Bayan Bilingual School, or at one of the Authority’s branches during the morning period from 8:00 a.m. until 1:00 p.m.​
